What it is, How it works

When taken near the scalp, it can reveal a detection period of up to roughly 3 months for alcohol and other substances. Hair is straightforward to collect, relatively challenging to tamper with, and easy to dispatch.

A sample of 1.5 inches consisting of around 200 hair strands (about the volume of a #2 pencil) nearest to the scalp will provide 100mg of hair, which is the optimal sample for testing and verification. For EtG, additional tests, and/or assays beyond 10 panels, it is advisable to gather 150mg of the specimen. Utilizing a jeweler’s scale for measuring the specimen is advisable. In instances where scalp hair is lacking, a similar volume of body hair can be gathered. The term head hair refers exclusively to scalp hair, while body hair pertains to all extra types of hair (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test outcome involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the preliminary handling of a sample into a laboratory’s framework. This step includes certifying that the sample was properly sealed and sent, allocating a random LAN (Laboratory Accessioning Number), and finalizing any further data entry not supplied by an electronic chain of custody framework.

Screening consists of an initial brief evaluation for illicit drug presence. While Screening is a cost-efficient procedure to exclude drug usage in the majority of samples, a positive result requires confirmation to be valid in court. Any samples that test presumptively positive in Screening mandate secondary confirmation.

Should a sample test presumptively positive in the Screening phase, additional hair is extracted from the original sample and readied for Extraction. At this stage, drugs are extracted from hair at considerably lower concentrations than with other methodologies (e.g., urine or oral fluid), highlighting why hair drug screening is the most challenging methodology to execute.

Confirmation of any positive screening result is performed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing prior to confirmation as needed. The entire laboratory process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
