A 1.5-inch section of approximately 200 hair strands (similar to a #2 pencil in size) nearest to the scalp yields 100mg of hair, which is the optimum sample for both screening and confirmation. For EtG, additional panels, and/or tests exceeding 10 panels, it is advisable to have a sample of 150mg. Utilizing a jeweler’s scale to weigh the specimen is suggested. If scalp hair is not an option, an equal measure of body hair can be gathered. When we mention head hair, it specifically refers to scalp hair. Body hair includes all other types of hair (like facial and axillary).

Process Overview

The laboratory processing of a drug test comprises four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial integration of a sample into a laboratory’s system, encompassing the confirmation of proper sealing and shipping of the sample, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry not provided by an electronic chain of custody system.

Screening involves an initial swift analysis for drugs of abuse. Although Screening is a cost-efficient method to eliminate drug usage for most samples, a positive screen needs to be confirmed for it to be legally acceptable. Any samples designated as presumptively positive in Screening necessitate a secondary confirmation.

Should a sample be identified as presumptively positive during the Screening stage, additional hair is extracted from the original specimen and prepared for Extraction. This stage involves extracting drugs from hair at a notably lower concentration than with other methodologies (e.g., urine or oral fluid), making hair drug analysis the most challenging methodology to perform.

Confirmation of any positive screening result is accomplished using GC/MS, GC/MS/MS, or LC/MS/MS. All samples initially testing positive are washed as required before confirmation. The entirety of the laboratory process, from Accessioning to Confirmation, is scrutinized under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
