By capturing biomarkers within the fibers of the growing strand, hair presents a prolonged record of alcohol and substance use. When collected near the scalp, hair provides nearly a 3-month detection period for various substances. It's easy to collect, challenging to tamper with, and simple to ship.

A 1.5-inch portion, comprising about 200 hair strands (equivalent to a #2 pencil's thickness) from nearest the scalp provides 100mg of sample, suitable for both screening and confirmation. For EtG, additional tests, or more than 10-panel screenings, 150mg is advised. We suggest using a jeweler's scale to weigh the sample. If scalp hair is absent, a comparable volume of body hair may be gathered. When referring to head hair, we mean only scalp hair. Body hair encompasses all other hair varieties (facial, axillary, etc.).

Process Overview

The main stages in a laboratory drug test processing include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial sample processing within a laboratory's framework. This involves ensuring proper sample sealing and transport, assigning a random LAN (Laboratory Accessioning Number), and completing supplementary data entry if absent from an electronic chain of custody system.

Screening provides a preliminary check for drugs of abuse. While it's an economical approach to dismiss drug presence in most samples, a positive screen needs verification for court validity. Any presumptively positive Screening samples require further confirmation.

If a sample indicates presumptive positivity in Screening, more hair is extracted from the initial sample and readied for Extraction. This phase involves extracting drugs from hair at lower concentrations than with other methodologies (e.g., urine or oral fluid), making hair drug testing the most complex to conduct.

Verification of any positive screening results is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation as needed. From Accessioning to Confirmation, the entire lab process is assessed under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
