For tests involving EtG, additions, or those exceeding 10 panels, a specimen weight of 150mg is preferable. It's advisable to use a jeweler’s scale for precise weighing. If scalp hair isn’t available, an equivalent quantity of body hair can be used. When mentioning head hair, we specifically mean scalp hair, while body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into the lab system. This process ensures the sample was securely sealed and transported, assigns a random LAN (Laboratory Accessioning Number), and completes any extra data entry missing from an electronic chain of custody system.

Screening conducts a rapid initial assessment for substance abuse. Although Screening efficiently eliminates drug usage for many samples, a positive result requires confirmation for legal validity. All samples that test presumptively positive in Screening need further confirmation.

Should a sample be presumptively positive during Screening, additional hair is sourced from the initial sample for the Extraction phase. Here, substances are retrieved from hair at notably lower concentrations than in other methods (e.g., urine or oral fluid), making hair drug screening the most challenging methodology to execute.

Confirmation of any positive Screening outcome is achieved using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positives are cleaned before confirmation as necessary. The complete laboratory procedure, from Accessioning to Confirmation, is examined under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
