A sample measuring 1.5 inches consisting of about 200 strands of hair (approximating the size of a #2 pencil) close to the scalp yields 100mg of hair, which is the preferred sample for screening and verification. For EtG, add-ons, and/or tests exceeding 10 panels, a 150mg specimen is suggested. It's advisable to measure the specimen using a jeweler's scale. In cases where scalp hair isn't available, an equivalent quantity of body hair can be collected. Head hair specifically refers to scalp hair, whereas body hair includes all other types (e.g., facial, axillary).

Process Overview

The primary stages in the laboratory processing of drug test outcomes include Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial handling of a sample in a lab's system. It encompasses validating that the sample was sealed and dispatched correctly, allocating a random LAN (Laboratory Accessioning Number), and performing any extra data input not provided by an electronic chain of custody system.

Screening entails a preliminary quick assessment for substance abuse. While Screening provides a cost-efficient approach to exclude drug usage for the majority of samples, a positive screen requires verification to be permissible in legal settings. Any samples showing presumptive positives in Screening must undergo a secondary confirmation.

If a sample indicates a presumptive positive status during the Screening phase, additional hair is extracted from the original specimen for preparation for Extraction. In this phase, substances are derived from hair at a much lower concentration than other methods (e.g., urine or oral fluid), highlighting the complexity of performing hair drug screening.

Confirmation of any positive screening findings is executed via GC/MS, GC/MS/MS, or LC/MS/MS. All samples showing presumptive positives are washed before confirmation as necessary. The complete lab process from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair classification as well as the ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
