If head hair cannot be obtained, a similar amount of body hair may be used. References to head hair concern only the scalp hair. Body hair encompasses all other types of hair (including facial, axillary, etc.).

The laboratory analysis for a drug test outcome includes four primary steps: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, the sample is initially entered into the laboratory system. This step involves checking that the sample is correctly sealed and delivered, assigning a random LAN (Laboratory Accessioning Number), and performing any further data entry not accounted for by an electronic chain of custody system.

Screening entails an initial rapid assessment for drug misuse. Although Screening is cost-efficient for excluding drug use in most samples, any positive findings require further confirmation to be valid legally. Presumptive positives from Screening need subsequent confirmation.

For samples testing presumptive positive during the Screening, additional hair is extracted from the beginning sample and set up for Extraction. Here, drugs are extracted from hair in concentrations substantially lower than those found in other methods (such as urine or oral fluid), making hair drug analysis particularly challenging.

Confirmation of any positive results from Screening is conducted via GC/MS, GC/MS/MS, or LC/MS/MS. All samples flagged as presumptive positive are cleaned before confirmation as necessary. The complete laboratory procedure from Accessioning to Confirmation undergoes examination under the CAP (College of American Pathologists) Hair classification and accreditation per ISO / IEC 17025 standards.
