When sourced from near the scalp, hair can depict up to a 3-month timeline for alcohol and drug presence. Hair collection is straightforward, challenging to tamper with, and simple to transport.

An inch-and-a-half section of roughly 200 hair strands (akin to a #2 pencil) closest to the scalp yields 100mg, which is optimal for testing. In cases involving EtG, extensions, and tests beyond 10 panels, 150mg of the sample is advisable. It’s suggested to weigh the sample with a jeweler’s scale. If scalp hair isn’t available, a body hair equivalent may be taken. When mentioning head hair, we mean only scalp hair. Body hair encompasses all other types (including facial, axillary, etc.).

Process Overview

There are four primary steps in processing a drug test result in a laboratory: Accessioning, Screening, Extraction, and Confirmation.

Accessioning starts with the sample's entry into the laboratory system. This involves verifying proper sealing and shipping of the sample, allocating a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry not included by an electronic chain of custody system.

Screening offers an initial cursory examination for drugs of abuse. While it is cost-efficient to exclude drug use in most samples during Screening, a positive result must be confirmed for legal proceedings. Samples that are tentatively positive in Screening necessitate a subsequent confirmation.

Samples testing tentatively positive in the Screening phase result in more hair being taken from the initial sample and prepped for Extraction. Here, drugs are extracted from the hair at significantly lower concentrations than with other types (e.g., urine or oral fluid), underpinning why hair drug testing is the most complex methodology.

Confirmation of positive screening results takes place through GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives undergo washing before confirmation as deemed necessary. The entire laboratory process from Accessioning to Confirmation is conducted following both the CAP (College of American Pathologists) Hair criteria and accreditation in line with ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
