What it is, How it works

Obtaining a 1.5-inch hair section with roughly 200 strands (comparable to the thickness of a #2 pencil) closest to the scalp yields about 100mg, the optimal sample for both screening and confirmation. For EtG, additional tests, or more than 10 panels, it's advised to have 150mg. Utilizing a jeweler’s scale for measurement is suggested. Should scalp hair be lacking, an equivalent weight of body hair can be used. Head hair specifically indicates scalp hair, while body hair encompasses all other types (such as facial or axillary).

Process Overview

The laboratory procedure for processing a drug test result consists of four principal steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails integrating a sample into the laboratory system. This step verifies that the sample has been properly sealed and shipped, assigns a random LAN (Laboratory Accessioning Number), and completes any necessary data entry not included in an electronic chain of custody system.

Screening offers an initial rapid examination for drugs of abuse. Although it is a cost-effective method to rule out drug consumption in most samples, a positive screening necessitates confirmation for legal acceptability. Any presumptively positive samples during Screening require a secondary confirmation.

If a Screening stage indicates a presumptive positive, additional hair is extracted from the initial collection for the Extraction phase. This phase isolates drugs from hair at notably lower concentrations than other methods, such as urine or oral fluids, attesting to the complexity of hair drug screening.

Confirmation of a positive screening result is performed with techniques like GC/MS, GC/MS/MS, or LC/MS/MS. All samples testing presumptive positive are cleansed prior to confirmation if needed. The complete laboratory process, from Accessioning to Confirmation, is conducted under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
