What it is, How it works

When obtained near the scalp, hair can reveal drug and alcohol consumption within roughly a three-month period. Hair is straightforward to gather, relatively immune to tampering, and simple to transport.

A 1.5-inch portion containing roughly 200 hair strands (approximately the thickness of a #2 pencil) closest to the scalp provides 100mg of hair, the optimal sample for analysis and verification. For EtG, add-ons, and/or analysis exceeding 10 panels, 150mg is advisable. We suggest using a jeweler’s scale for precise measurement. In the absence of scalp hair, an equivalent quantity of body hair may be procured. The term head hair exclusively pertains to scalp hair, while body hair encompasses all other types (such as facial and axillary).

Process Overview

The drug test result laboratory processing involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the primary introduction of a sample into the laboratory’s system. This stage ensures the sample is properly sealed and dispatched, assigns an arbitrary LAN (Laboratory Accessioning Number), and completes any additional data entry absent from an electronic chain of custody framework.

Screening consists of an expedited assay for substance abuse. Although Screening is a budget-friendly approach to exclude drug usage in most samples, the results need validation to be legally acceptable. Any samples showing an initial positive in Screening mandate further confirmation.

When a sample shows a presumptive positive in Screening, more hair is extracted from the preliminary specimen for Extraction. This phase involves extracting drugs from hair at significantly lower concentrations compared to other techniques (e.g., urine or oral fluid). Consequently, hair drug testing is the most challenging methodology to execute.

Confirmation of positive screen results is carried out using GC/MS, GC/MS/MS, or LC/MS/MS methods. Samples that presumptively test positive are cleansed prior to confirmation as necessary. The complete lab operation from Accessioning to Confirmation complies with CAP (College of American Pathologists) Hair parameters and follows ISO / IEC 17025 standard accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
