Should scalp hair be unavailable, an equivalent quantity of body hair can be collected. References to head hair pertain exclusively to scalp hair, while body hair encompasses all other types (e.g., facial, axillary).

Process Overview

The laboratory workflow for processing a drug test result includes these four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning concerns the preliminary processing of a sample within a lab’s system. This step verifies that the sample has been properly sealed and transported, assigns a random LAN (Laboratory Accessioning Number), and completes any missing data entry that an electronic chain of custody system does not address.

Screening serves as an initial swift assessment for drugs of abuse. Although cost-effective for excluding drug use in most samples, a positive screen must be confirmed for it to stand in legal proceedings. Samples with presumptive positive results at the Screening phase require a follow-up confirmation.

When a sample shows a presumptive positive result during Screening, additional hair is taken from the original specimen and processed for Extraction. During Extraction, drugs are isolated from the hair at significantly lower concentrations compared to methods like urine or oral fluid testing, making hair drug screening particularly challenging.

Confirmation of any positive screening result is performed via GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positive outcomes are washed before confirmation, as necessary. The entire laboratory sequence from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and accredited to ISO / IEC 17025 standards.
