A hair sample of 1.5 inches, approximately 200 strands or the diameter of a #2 pencil, from near the scalp provides about 100mg of hair—the perfect quantity for both screening and confirmation. For EtG, additional tests, or screenings over 10 panels, around 150mg of the sample is advisable. We suggest weighing the sample on a jeweler’s scale. If scalp hair isn’t available, a comparable amount of body hair can be used. "Head hair" exclusively refers to scalp hair, while "body hair" encompasses other types such as facial and axillary hair.

Process Overview

The laboratory analysis of a drug test result includes four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the process of registering a sample in the lab’s system, which involves ensuring the sample was properly sealed and delivered, generating a random LAN (Laboratory Accessioning Number), and filling out any missing information not captured electronically.

Screening entails an initial rapid assessment for the presence of drugs. Although Screening provides a cost-efficient means to detect drug use in most samples, a positive screening part requires confirmation to hold up in legal settings. Samples that are presumptively positive in Screening do need a follow-up with confirmation.

If Screening identifies a sample as presumptively positive, additional hair is extracted from the original sample and readied for Extraction. Here, drugs are isolated from hair at significantly lower concentrations compared to other methods (such as urine or saliva), making hair drug screening a complex procedure.

Confirmation of a positive screen is performed using GC/MS, GC/MS/MS, or LC/MS/MS techniques. Samples that test presumptive positive are cleaned as needed before confirmation. The complete laboratory procedure, from Accessioning to Confirmation, is subject to review under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
