What it is, How it works

Testing hair is now recognized as an effective method for identifying alcohol and drug usage. Hair captures biomarkers within the fibers of the hair shaft, presenting a prolonged record of alcohol and drug exposure. When cut near the scalp, it can offer a detection timeframe of up to roughly three months for substances. Hair collection is straightforward, relatively tamper-proof, and simple to transport.

A sample measuring 1.5 inches, consisting of approximately 200 hair strands (similar in size to a #2 pencil) near the scalp, yields 100mg of hair, which is optimal for both screening and confirmation. For EtG, additional tests, and/or tests exceeding 10 panels, it is advised to collect 150mg of the sample. Using a jeweler’s scale to measure the sample weight is suggested. If scalp hair isn’t available, a comparable amount of body hair may be used. By head hair, we mean only scalp hair; body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory processing for a drug test result involves four primary steps, namely Accessioning, Screening, Extraction, and Confirmation.

Accessioning signifies the initial incorporation of a sample into a lab system. This involves confirming the proper sealing and shipping of the sample, assigning a random LAN (Laboratory Accessioning Number), and completing any further data entry not supplied by an electronic chain of custody system.

Screening consists of a preliminary swift assessment for drugs of abuse. While Screening is a cost-efficient approach to eliminate drug use for most samples, a favorable screen necessitates verification for court-admissible results. Any samples deemed presumptively positive in Screening require further confirmation.

If a sample is deemed presumptively positive during Screening, additional hair is extracted from the initial sample for Extraction. Within this phase, drugs are extracted from hair at significantly lower concentrations than alternative methods (e.g., urine or oral fluid), rendering hair drug testing the most challenging methodology to execute.

Confirmation of any positive screening outcome is administered via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation as required. The laboratory process from Accessioning through Confirmation is examined both under the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
