A sample measuring 1.5 inches, consisting of approximately 200 hair strands (akin to a #2 pencil's thickness) nearest to the scalp provides 100mg of hair, which is the preferred amount for both screening and confirmation. For EtG, additional panels over 10, 150mg is advised. We suggest using a jeweler’s scale for precise specimen weighing. In the absence of scalp hair, a similar volume of body hair can be utilized. Head hair strictly refers to scalp hair, while body hair encompasses all other types (facial, axillary, etc.).

Process Overview

There are four primary phases in the lab processing of drug tests: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the introductory handling of a sample within the lab's records. This process ensures the sample was correctly sealed and sent, assigns a unique LAN (Laboratory Accessioning Number), and finalizes any extra data entry omitted by an electronic chain of custody system.

Screening involves conducting a preliminary assessment for drugs. Though Screening is an economical means to rule out drug use in most samples, a positive screen necessitates confirmation to hold in court. Presumptive positives in Screening mandate subsequent confirmation.

For samples that show a tentative positive during Screening, additional hair is drawn from the initial sample and readied for Extraction. Drugs are extracted from hair at a notably lower concentration compared to other methods (e.g., urine or oral fluid), rendering hair drug analysis the most challenging to execute.

The cross-verification of any positive screening outcome is done using GC/MS, GC/MS/MS, or LC/MS/MS. All initially positive samples undergo a wash before confirmation as required. The comprehensive lab process, from Accessioning to Confirmation, undergoes scrutiny under the CAP (College of American Pathologists) Hair designation and holds accreditation to ISO / IEC 17025 standards.

In this special issue of Drug Testing and Analysis, we aim to showcase the remarkable manuscripts presented at this significant event, shedding light on the latest research and highlighting the future directions in this field.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
