What it is, How it works

If collected near the scalp, hair can reveal a detection window of up to about 90 days for alcohol and drugs. Hair collection is straightforward, somewhat resistant to tampering, and easy to transport.

A 1.5-inch section of roughly 200 strands (comparable to a #2 pencil in width) nearest to the scalp yields 100mg of hair, ideal for screening and verification. For EtG, add-ons, and/or examinations exceeding 10 panels, 150mg of hair is advised. It is suggested to weigh the sample on a jeweler’s scale. In the absence of scalp hair, a similar quantity of body hair might be gathered. When discussing head hair, we solely refer to scalp hair. Body hair denotes all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result consists of four main steps: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample undergoes initial processing into the lab’s system. This step involves verifying proper sealing and shipping of the sample, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry not captured by an electronic chain of custody system.

The Screening phase is a preliminary rapid assessment for drug misuse. While Screening efficiently clears negative results for the majority of samples, a positive screen requires confirmation for court usage. Thus, any preliminary positives in Screening need further verification.

If a sample tests presumptively positive during Screening, more hair is drawn from the original sample for Extraction. This phase involves extracting drugs from hair at minute concentrations, unlike other methods (e.g., urine or oral fluid), making hair drug screening the most challenging to execute.

The GC/MS, GC/MS/MS, or LC/MS/MS techniques confirm any positive screening outcomes. All samples marked presumpitvely positive are rinsed before confirmation if necessary. The laboratory procedure from Accessioning to Confirmation undergoes inspection under CAP (College of American Pathologists) Hair criteria and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
