What it is, How it works

Hair testing is now recognized as an effective method for identifying drug and alcohol consumption. Hair offers a long-term record of alcohol and drug usage by embedding biomarkers within the hair's growing strands. When taken close to the scalp, hair can indicate drug and alcohol usage for up to three months. Hair is easy to collect, relatively hard to tamper with, and convenient to transport.

A 1.5-inch sample of roughly 200 hair strands (similar to the width of a #2 pencil) nearest the scalp yields 100mg of hair, the optimal amount for screening and confirmation. For EtG, additional analytes, and/or tests exceeding 10 panels, 150mg is suggested. We suggest using a jeweler’s scale for weighing the sample. If scalp hair is not accessible, a similar amount of body hair may be collected. By head hair, we mean scalp hair specifically. Body hair encompasses all other types of hair (e.g., facial, axillary).

Process Overview

The laboratory processing of drug test results encompasses four main steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves integrating a sample into a laboratory’s system. This process confirms the sample was correctly sealed and transported, assigns a random LAN (Laboratory Accessioning Number), and completes any data entry not available through an electronic chain of custody system.

Screening is an initial rapid check for drug abuse. Although Screening is a cost-efficient way to discount drug usage in most samples, a positive screen requires confirmation to be court-admissible. Presumptive positive samples in Screening need confirmation.

In the event of a presumptive positive result in Screening, additional hair is extracted from the first sample and set for Extraction. At this stage, drugs are retrieved from hair at lower concentrations than in other methods (e.g., urine or oral fluid), marking hair drug screening as a complex method.

Positive screening results are confirmed via GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples are cleaned before required confirmation. The laboratory process undergoes a review under CAP (College of American Pathologists) Hair requirements and ISO / IEC 17025 accreditation standards.

Advantages of hair drug testing:

  • Extended detection period: Hair drug tests can capture drug usage for up to 90 days, unlike urine tests, which have a more limited detection span.
  • Difficult to deceive: It's nearly impossible to falsify a hair drug test, thereby enhancing accuracy.
  • Provides historical insight: It can reveal a trend of drug use over a prolonged timeframe, not just recent use.

Limitations:

  • Incapable of detecting recent use: Drugs take about 5-7 days to become evident in hair.
  • Cost: Hair drug tests generally incur higher costs than other testing methods.
  • Result variability: Elements like hair color and individual differences in hair growth can alter drug metabolite levels in the hair.

Note: Though commonly termed "hair follicle tests," the examination analyzes the hair strand instead of the follicle beneath the scalp.
