What it is, How it works

Hair analysis is widely recognized as an effective instrument for uncovering drug and alcohol consumption. Hair captures a comprehensive history of substance use by embedding biomarkers in the threads of the hair as it grows. Collecting hair near the scalp can offer a detection period of up to roughly three months for substances. Hair collection is straightforward, less susceptible to tampering, and easy to transport.

A 1.5-inch hair sample, consisting of around 200 strands (about the thickness of a #2 pencil) taken nearest to the scalp yields 100mg, the optimal amount for testing and verification. For EtG, additional tests, and tests beyond 10 panels, 150mg of the hair is suggested. Using a jeweler’s scale to weigh the sample is advised. If scalp hair is not obtainable, a comparable amount of body hair can be used. Head hair references are specifically to scalp hair. Body hair pertains to all other hair types (facial, axillary, etc.).

Procedural Summary

The essential phases in the lab processing of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial entry of a sample into the laboratory's system. This process ensures the sample was sealed and mailed correctly, assigns a random LAN (Laboratory Accessioning Number), and completes any additional data input not covered by an electronic chain of custody system.

Screening consists of a preliminary rapid scan for drugs of abuse. While Screening is an economical way to eliminate drug use in the majority of samples, a positive result must be confirmed to hold up in legal situations. Samples that initially test positive in Screening need additional confirmation.

In the event a sample tests positive initially, additional hair is taken from the original specimen and readied for Extraction. At this point, drugs are extracted from hair at significantly lower concentrations than with other methods (e.g. urine or oral fluid), which is why the hair drug analysis is the most complex method to execute.

Confirming any positive result from the screener is done through GC/MS, GC/MS/MS, or LC/MS/MS. All preliminary positive samples are cleaned prior to confirmation if necessary. The entirety of the lab procedure from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair designation and is accredited to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
