What it is, How it works

Hair captures a prolonged record of alcohol and drugs by encapsulating biomarkers within the strands' fibers during growth. When harvested near the scalp, hair can offer up to a three-month detection period for alcohol and drugs. The collection of hair is straightforward, relatively challenging to tamper with, and convenient to transport.

A 1.5-inch section comprising roughly 200 hair strands (approximately the width of a #2 pencil) nearest to the scalp will yield 100mg of hair, which is the optimal sample quantity for evaluation and validation. For EtG, additional elements, and/or examinations exceeding 10 panels, 150mg of the sample is advisable. It's suggested to measure the sample using a jeweler’s scale. If head hair is not an option, a comparable quantity of body hair can be gathered. When mentioning head hair, we specifically mean scalp hair. Body hair pertains to all other kinds such as facial, axillary, etc.

Process Overview

The core steps in processing a drug test in the lab are Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the initial processing of a sample into the lab's system. This step verifies that the sample was properly sealed and transported, assigns a random LAN (Laboratory Accessioning Number), and concludes any remaining data entry not covered by an electronic chain of custody system.

Screening provides a preliminary assessment for illicit drugs. While it’s a cost-efficient method to eliminate drug usage in most samples, a positive result from Screening must undergo confirmation for it to be court-admissible. Samples indicated as possibly positive in Screening need further verification.

If a sample shows a possible positive result during the Screening phase, additional hair is extracted from the initial sample for the Extraction phase. In Extraction, drugs are retrieved from hair at significantly lower concentrations than other techniques (e.g., urine or oral fluid), which is why hair drug testing poses the greatest challenge.

GC/MS, GC/MS/MS, or LC/MS/MS methods confirm any positive results after screening. All samples possibly positive are cleansed prior to confirmation as required. The entire lab process from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
