What it is, How it works

A sample of 1.5 inches consisting of roughly 200 hair strands, comparable in size to a #2 pencil, closely cropped to the scalp, yields 100 mg of hair, the optimal quantity for screening and confirmation processes. For EtG, additional screenings, or tests exceeding ten panels, it's advised to collect 150 mg of the specimen. It's suggested to measure the specimen using a jeweler's scale. In the absence of scalp hair, an equivalent quantity of body hair can be utilized. When mentioning head hair, it refers solely to scalp hair. Body hair encompasses all other types (such as facial, axillary, etc.).

Process Overview

The lab processing of a drug test result is comprised of four essential steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning marks the beginning of processing a sample into the lab's system. This phase ensures the sample is sealed and transported correctly, assigns a unique LAN (Laboratory Accessioning Number), and completes any further data input required beyond what is supplied by an electronic chain of custody system.

Screening offers a preliminary, efficient evaluation for substances of abuse. While cost-effective, a positive screen necessitates further confirmation to hold up legally. Samples that are possibly positive during Screening require a subsequent validation.

If Screening indicates a presumptive positive result, additional hair is extracted from the original specimen and readied for Extraction. Drugs are isolated from hair at significantly lower concentrations than with other methods (like urine or oral fluid), highlighting hair drug screening's complexity.

Positive results from Screening undergo Confirmation using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positive outcomes are cleansed before confirmation as needed. The entire process from Accessioning to Confirmation is subject to review under the CAP (College of American Pathologists) Hair certification and ISO/IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
