For EtG, additional tests, and/or panels exceeding 10, a 150mg sample is advisable. We suggest weighing the sample on a jeweler’s scale. In cases where scalp hair is not present, an equivalent quantity of body hair may be obtained. The term head hair specifically refers to scalp hair. Other hair types, such as facial or axillary, fall under the category of body hair.

Process Overview

The laboratory processing of a drug test encompasses four principal stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial sample processing within a laboratory system. This step includes confirming the sample's proper sealing and shipment, assigning a random Laboratory Accessioning Number (LAN), and completing any necessary additional data entry absent from an electronic chain of custody system.

Screening involves a preliminary rapid examination for drugs of abuse. While Screening serves as a cost-efficient means to exclude drug use in most cases, confirmatory testing is required to ensure results are court-admissible. Samples that yield a presumptive positive in Screening mandate further verification.

Should a sample test presumptively positive during Screening, additional hair is extracted from the initial sample and readied for Extraction. During this stage, drugs are extracted from hair at much lower concentrations compared to other methods (such as urine or oral fluid), highlighting why hair drug screening is among the most challenging methodologies to execute.

GC/MS, GC/MS/MS, or LC/MS/MS technologies are employed for confirming any positive screening outcomes. Presumptive positives are thoroughly washed before confirmation as necessary. The entire laboratory procedure, from Accessioning through to Confirmation, is conducted under both CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
