A 1.5-inch segment of around 200 hair strands (roughly a #2 pencil's width) nearest the scalp yields 100mg of hair, the ideal sample for both screening and confirmation. For EtG, additional tests, and/or those exceeding 10 panels, 150mg is advised. We suggest using a jeweler’s scale to weigh the sample. If scalp hair isn’t accessible, a similar quantity of body hair can be taken. Regarding head hair, we mean scalp hair exclusively. Body hair encompasses all other types (face, underarm, etc.).

Process Overview

The laboratory processing of a drug test result primarily comprises four stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step of processing a sample into a lab system. This ensures the sample was properly sealed and transported, assigns a random LAN (Laboratory Accessioning Number), and completes any further data entry absent from an electronic chain of custody system.

Screening performs an initial swift assessment for drugs. Though cost-effective for ruling out drugs in most samples, positive screens must be confirmed to be legitimate in court. All samples that test presumptively positive in Screening necessitate subsequent confirmation.

If Screening yields a presumptive positive, a larger portion of hair is drawn from the original specimen and prepped for Extraction. Drugs are extracted from hair at notably lower concentrations than other methods (such as urine or saliva), rendering hair drug screening the most intricate methodology.

Confirmation of positive screens is executed using GC/MS, GC/MS/MS or LC/MS/MS techniques. Presumptive positive samples undergo washing before confirmation if required. From Accessioning to Confirmation, every step is scrutinized under the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field.
