For EtG tests, additional panels, and/or tests exceeding 10 panels, a specimen weighing 150mg is suggested. It is advised to weigh the specimen using a jeweler’s scale. If scalp hair cannot be collected, an equivalent amount of body hair may be used. "Head hair" specifically refers to scalp hair. "Body hair" encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step of processing a sample into the laboratory system. This step includes verifying that the sample is properly sealed and shipped, assigning a unique LAN (Laboratory Accessioning Number), and completing any remaining data entry not handled by an electronic chain of custody system.

Screening is an initial, rapid check for drugs of abuse. It is a cost-effective method for ruling out drug use in most samples; however, a positive result requires confirmation to be valid in legal settings. Any samples that yield a presumptive positive in Screening must undergo further confirmation.

If the Screening stage results in a presumptive positive, additional hair is extracted from the original specimen for the Extraction process. In this stage, drugs are extracted at much lower concentrations from hair than from other samples (e.g., urine or oral fluid), making hair drug testing the most challenging methodology.

Any positive screening result must be confirmed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples that are presumptively positive are washed prior to confirmation as necessary. The entire process from Accessioning to Confirmation is reviewed under both the CAP (College of American Pathologists) Hair designation and accredited to ISO / IEC 17025 standards.
