A sample measuring 1.5 inches, consisting of roughly 200 hair strands (akin to the thickness of a #2 pencil) nearest the scalp will yield 100mg of hair, which is the optimal quantity for both screening and confirmation. For EtG, add-ons, and/or panels exceeding 10, 150mg of the sample is advisable. Using a jeweler’s scale for weighing the specimen is recommended. Should scalp hair not be obtainable, an equivalent amount of body hair can be used. "Head hair" specifically denotes scalp hair, whereas "body hair" encompasses all other hair types (e.g., facial, axillary).

Process Overview

The laboratory processing for a drug test result encompasses four primary phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the entry of a sample into a laboratory’s framework, including confirming the sample’s proper sealing and dispatch, assigning a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry absent from an electronic chain of custody system.

Screening serves as an initial rapid evaluation for substance abuse. Though it is a cost-efficient method to exclude drug usage in most samples, positive screenings require confirmation for legal admissibility. Samples that appear presumptively positive during Screening necessitate secondary confirmation.

For samples identified as presumptively positive in the Screening stage, additional hair is extracted from the initial specimen for preparation in Extraction. Here, drug extraction from hair occurs at much lower concentrations compared to methods like urine or oral fluid, ultimately rendering hair drug screening as the most challenging methodology to execute.

Confirmation of any positive screening result utilizes GC/MS, GC/MS/MS, or LC/MS/MS technology. Necessary washing of all presumptively positive samples precedes confirmation. The complete laboratory process from Accessioning to Confirmation is meticulously reviewed under the CAP (College of American Pathologists) Hair designation combined with accreditation to ISO / IEC 17025 standards.
