A 1.5-inch segment of approximately 200 hairs (similar in size to a #2 pencil) closest to the scalp yields 100mg of hair, the optimal sample for analysis and verification. For EtG, additional tests, and/or those beyond 10 panels, 150mg of the sample is advisable. We suggest weighing the specimen with a jeweler’s scale. If scalp hair is absent, a comparable amount of body hair can be used. By head hair, we mean scalp hair exclusively. Body hair encompasses all other hair types (such as facial, axillary, etc.).

Process Overview

The fundamental steps in laboratory processing of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the preliminary handling of a sample into a lab's system. This comprises ensuring the sample was properly sealed and dispatched, allocating a random LAN (Laboratory Accessioning Number), and finishing any extra data entry not managed by an electronic chain of custody framework.

Screening entails an initial brief check for substances of abuse. Although Screening is an economical method for excluding drug usage in most samples, a positive screen requires validation to be accepted in legal settings. Presumptive positive samples in Screening necessitate a subsequent confirmation.

If a sample shows presumptive positive results during Screening, additional hair is extracted from the original sample for Extraction. This phase involves extracting substances from hair at significantly lower concentrations compared to other methods (e.g., urine or oral fluid), making hair drug screening the most challenging method to conduct.

Confirmation of any positive screening result is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation as required. The complete process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.
