A 1.5-inch section of about 200 strands of hair, roughly the diameter of a #2 pencil, taken closest to the scalp yields 100mg, which is optimal for screening and confirmatory tests. For tests like EtG, additional screens, or those beyond 10 panels, a specimen amounting to 150mg is advised. We recommend utilizing a jeweler’s scale to weigh the specimen accurately. In cases where scalp hair is scarce, an equivalent portion of body hair may be utilized. Here, “head hair” denotes scalp hair specifically, while “body hair” encompasses all other types, such as facial and axillary hair.

Process Overview

The laboratory processing of a drug test result includes four key phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the initial integration of a sample into the lab’s system, which encompasses verifying proper sealing and shipment of the sample, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data input absent from an electronic chain of custody system.

Screening consists of a preliminary check for substance abuse. While Screening is economical, serving to eliminate drug usage in most samples, a positive outcome must be substantiated to be court admissible. Samples yielding a presumptive positive in Screening necessitate follow-up confirmation.

For presumptively positive samples in the Screening phase, additional hair is drawn from the original sample for Extraction. This phase extracts drugs from hair at concentrations lower than those in other methods (e.g., urine or oral fluid), hence hair drug testing is acknowledged as a complex methodology.

Confirming any positive Screening results employs GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, all samples with presumptive positives undergo washing as required. The complete laboratory procedure from Accessioning to Confirmation adheres to both the CAP (College of American Pathologists) Hair standards and the accreditation of ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
