What it is, How it works

Hair captures a prolonged history of substance use by embedding markers within the strands as they grow. When taken near the scalp, a hair sample can offer around a 3-month period for detecting alcohol and drug presence. Collecting hair is straightforward, challenging to tamper with, and convenient to send.

A sample of 1.5 inches comprised of roughly 200 hair strands (similar to a #2 pencil) near the scalp yields 100mg, which is ideal for both screening and confirmation tests. For EtG, additional tests, and instances exceeding 10 panels, it is suggested to have 150mg. We suggest using a jeweler’s scale for measurement. If scalp hair isn't available, equivalently, body hair can be used. Head hair specifically refers to hair on the scalp. Body hair encompasses all other hair varieties (such as facial, axillary, etc.).

Process Overview

Lab processing entails four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the sample's initial processing phase into the laboratory system. It involves verifying proper sample sealing and shipping, assigning a random LAN (Laboratory Accessioning Number), and completing any additional documentation not supplied by an electronic chain of custody system.

Screening provides a preliminary drug check. While it is a cost-efficient method to rule out drug use for most samples, a positive finding requires verification to be legal in court. Samples that are presumably positive during Screening need a secondary validation.

For samples deemed presumptively positive during Screening, additional hair is taken from the original sample for Extraction. Here, drugs are retrieved from hair in smaller concentrations compared to other methods (such as urine or oral fluid), making hair drug screening the most challenging to execute.

Confirmation of any positive screen outcomes is achieved through GC/MS, GC/MS/MS, or LC/MS/MS. All samples presumed positive are washed before confirmation as necessary. The complete process from Accessioning to Confirmation is audited under both the CAP (College of American Pathologists) Hair designation along with the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
