Accessioning is the initial stage where a sample is entered into a laboratory’s system. This step involves verifying that the sample was properly sealed and sent, assigning a unique LAN (Laboratory Accessioning Number), and completing any necessary data entry not covered by an electronic chain of custody system.
Screening is a preliminary examination for drugs of abuse. While Screening is an economical way to eliminate drug use in many samples, a positive screen must be confirmed to be valid for legal proceedings. Any samples deemed presumptively positive in Screening necessitate further confirmation.
If a sample is deemed presumptively positive during the Screening phase, additional hair is extracted from the initial sample and readied for Extraction. At this stage, drugs are isolated from hair at significantly lower concentrations than other methods (such as urine or oral fluid), which is why hair drug testing is the most complex method to undertake.
Confirmation of any positive screening result is achieved via GC/MS, GC/MS/MS, or LC/MS/MS. All samples testing as presumptively positive are washed prior to confirmation if required. The full lab process from Accessioning to Confirmation is overseen under the CAP (College of American Pathologists) Hair designation and adherence to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
