A sample measuring 1.5 inches, comprising roughly 200 strands of hair (similar to the girth of a #2 pencil) nearest the scalp, yields 100mg of hair, the optimal sample amount for testing and validation. For EtG, additional tests, and/or screenings beyond 10 panels, it is advised to use 150mg of the sample. It's suggested to measure the sample using a jeweler’s scale. If scalp hair cannot be obtained, an equivalent quantity of body hair can be used. "Head hair" refers specifically to scalp hair, whereas "body hair" includes all other types (facial, axillary, etc.).

Summary of Process

The laboratory handling of a drug test result comprises four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the initial intake of a sample into a lab’s system. The process ensures the sample was correctly sealed and shipped, allocates a unique LAN (Laboratory Accessioning Number), and handles any supplementary data input that an electronic chain of custody system might not cover.

Screening conducts a preliminary assessment for substance abuse. Confirmation for any positive Screening outcome is achieved using GC/MS, GC/MS/MS, or LC/MS/MS techniques. Presumptive positive samples undergo necessary washing before confirmation. The complete lab procedure, from Accessioning to Confirmation, is examined following both the CAP (College of American Pathologists) Hair criteria and the accreditation for ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
