A segment measuring 1.5 inches of approximately 200 hair strands (similar to the thickness of a #2 pencil) nearest the scalp will yield 100mg of hair, which is perfect for analysis and confirmation. For EtG, additional substances, and/or panels above 10, a 150mg amount is suggested. We advise using a jeweler’s scale to weigh the sample. In the case where scalp hair isn't available, a similar quantity of body hair can be collected. "Head hair" pertains solely to scalp hair, while "body hair" includes all other hair classifications (facial, axilla, etc.).

Process Overview

The four primary phases in the lab handling of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning consists of the primary processing of a sample into a lab's records. This encompasses confirming proper sealing and shipment of the sample, assigning it a unique LAN (Laboratory Accessioning Number), and completing any supplementary data entry absent in an electronic chain of custody system.

Screening is a preliminary rapid assessment for misuse of drugs. Though Screening is budget-friendly for dismissing potential drug usage in most samples, a positive result necessitates confirmation to be court-admissible. Samples suggested positive in Screening must undergo additional confirmation.

Should a sample appear positive in Screening, extra hair is retrieved from the original sample and arranged for Extraction. During this phase, substances are separated from hair in much lower concentrations compared to other methods (urine or oral fluid, for instance), making hair drug testing the most complex to execute.

GC/MS, GC/MS/MS, or LC/MS/MS methods verify any positive screening outcome during Confirmation. Presumptively positive samples undergo washing as required before confirmation. The full lab sequence from Accessioning through to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair classification and the ISO/IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
