A 1.5-inch segment of around 200 hair strands (roughly the thickness of a #2 pencil) taken nearest the scalp yields 100mg of hair, the optimal sample size for preliminary and confirmatory analysis. For EtG, add-ons, or tests exceeding 10 panels, 150mg of the hair sample is advised. We suggest using a jeweler’s scale to determine the specimen's weight. If scalp hair isn't available, a comparable quantity of body hair may be used. When mentioning head hair, we refer exclusively to scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The primary steps in lab processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves initiating the sample processing into the lab's system. This step includes ensuring the sample was correctly sealed and transported, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data entry not available via an electronic chain of custody system.

Screening is a preliminary rapid drug presence check. Although Screening effectively rules out drug use in most samples cost-efficiently, a positive result in Screening needs confirmation to be valid in court. Presumptively positive Screening samples require additional verification.

If a sample tests presumptively positive during the Screening phase, additional hair is extracted from the original specimen and prepped for Extraction. This phase involves extracting drugs from hair at much lower concentrations than alternative methods (such as urine or oral fluid), making hair drug testing the most complex method to perform.

Confirmation of any positive Screening result is done using GC/MS, GC/MS/MS or LC/MS/MS techniques. All samples that are presumptively positive get washed before Confirmation as required. The full lab process, from Accessioning to Confirmation, is evaluated according to both the CAP (College of American Pathologists) Hair criteria and accreditation in line with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
