For EtG, additional parameters, and/or tests exceeding 10 panels, a 150mg sample is advised. Using a jeweler’s scale for weighing the sample is recommended. If scalp hair is not available, a similar quantity of body hair can be used. When we mention head hair, it pertains exclusively to scalp hair. Conversely, body hair encompasses all other hair categories (facial, axillary, etc.).

Process Overview

The laboratory procedure for deriving a drug test result encompasses four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning denotes the initial processing phase of a sample into a laboratory’s database. This phase entails ensuring that the sample was sealed and transmitted accurately, assigning an arbitrary LAN (Laboratory Accessioning Number), and completing any further data entry absent from an electronic chain of custody framework.

Screening represents a preliminary swift evaluation for substances of abuse. Although Screening serves as a cost-effective strategy to deduce drug usage for most samples, verifying a positive screen is necessary for it to be judicially acceptable. Samples presumed to be positive during Screening demand a subsequent affirmation.

Subsequently, if a sample is identified as presumptively positive in Screening, additional hair is extracted from the original specimen for Extraction preparation. In this segment, drugs are extracted from hair at substantially lower concentrations compared to other techniques (e.g., urine or oral fluid), thus making hair drug screening the most complex method to execute.

To confirm any positive screening result, techniques such as GC/MS, GC/MS/MS, or LC/MS/MS are employed. Any presumptive positive samples undergo a cleaning procedure prior to confirmation if necessary. The comprehensive laboratory process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and accredited to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
