What it is, How it works

Hair testing is regarded as an effective means for detecting drug and alcohol consumption. Hair offers an extended history of substance usage by capturing biomarkers within the growing strand's fibers. When collected near the scalp, hair allows for a detection period of up to around 3 months for alcohol and drugs. Hair is straightforward to collect, relatively challenging to tamper with, and convenient to transport.

A sample measuring 1.5 inches consisting of roughly 200 hair strands (comparable to a #2 pencil's size) near the scalp will yield 100mg, which is the optimal quantity for both screening and confirmation. For EtG, additional tests, or tests exceeding 10 panels, 150mg of the sample is advised. We suggest using a jeweler’s scale to weigh the specimen. In the absence of scalp hair, an equivalent portion of body hair may be used. Referencing head hair pertains solely to scalp hair. Body hair includes all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test encompasses four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial sample processing within a lab's system. This process includes checking the sample was properly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and finalizing any extra data entry not covered by an electronic chain of custody system.

Screening offers an initial rapid assessment for substances of abuse. While cost-efficient for dismissing drug usage in most samples, positive screens necessitate confirmation for legal admissibility. Samples with presumptive positives in Screening require additional confirmation.

For presumptive positives in the Screening phase, additional hair is extracted from the specimen and prepped for Extraction. Drugs are extracted from hair at significantly lower concentrations than in other methodologies (e.g., urine or oral fluid), thereby making hair drug screening the most challenging methodology.

Confirmation of a positive screening is done via GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, presumptive positive samples are washed as necessary. The full laboratory process from Accessioning to Confirmation is under review by both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.
