A 1.5-inch section of about 200 hair strands (similar to the size of a #2 pencil) nearest to the scalp will yield 100mg of hair, the optimal example for evaluation and confirmation. For EtG, added tests, and/or examinations exceeding 10 panels, 150mg of the sample is advised. Utilizing a jeweler's scale for weighing the sample is recommended. If scalp hair cannot be acquired, a similar quantity of body hair may be utilized. By referring to head hair, we mean scalp hair exclusively. Body hair pertains to all other types (facial, axillary, etc.).

Process Overview

The primary phases in lab processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step of processing a sample within a laboratory’s framework. This process includes confirming the sample's proper sealing and shipping, allocating a random LAN (Laboratory Accessioning Number), and fulfilling any remaining data entry not covered by an electronic chain of custody system.

Screening entails a preliminary quick assessment for drugs of abuse. Although Screening is a budget-friendly method to exclude drug usage in most cases, a positive screen must be verified to be court-admissible. Any samples that test presumptively positive in Screening necessitate a secondary confirmation.

If a sample tests as presumptively positive during Screening, additional hair is extracted from the original specimen and prepared for Extraction. In this phase, drugs are extracted from hair at a significantly lower concentration compared to other approaches (e.g., urine or oral fluid), making hair drug screening the most challenging technique to execute.

Confirmation of any positive screening result is achieved via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed before confirmation if necessary. The comprehensive laboratory workflow from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair classification and the ISO / IEC 17025 standards accreditation.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
