What it is, How it works

Hair testing is now recognized as an effective method for identifying drug and alcohol consumption. Hair retains a long-term record of alcohol and drug usage by capturing biomarkers within the fiber of the growing hair strand. When sourced near the scalp, hair can offer close to a 3-month detection period for alcohol and drugs. Collection is easy, tampering is relatively tough, and shipping is effortless.

Collecting a 1.5-inch segment of approximately 200 hair strands (comparable to the width of a #2 pencil) nearest to the scalp yields 100mg, the ideal amount for screening and confirmation. For EtG, additional tests, and/or tests exceeding 10 panels, a 150mg specimen is suggested. It is advisable to weigh the specimen using a jeweler’s scale. If scalp hair is not accessible, an equivalent quantity of body hair may be used. Scalp hair specifically refers to hair on the head, whereas body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The primary steps in processing a lab drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning covers the initial processing of a sample into a lab's system. This process verifies proper sealing and shipping of the sample, assigns a random LAN (Laboratory Accessioning Number), and completes any additional data entry absent from the electronic chain of custody.

Screening entails an initial rapid check for drug abuse. Despite being cost-effective in ruling out drug use for most samples, a positive Screening result must be confirmed for court validity. Presumptively positive samples in Screening require subsequent confirmation.

Should a sample be presumptively positive in Screening, more hair is taken from the initial specimen for Extraction preparation. This stage involves extracting drugs from hair at significantly lower concentrations than other methods (e.g., urine or oral fluid), contributing to the complexity of hair drug screening.

Confirmation of any positive screening is handled via GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, all presumptive positive samples are cleansed as needed. Each phase from Accessioning to Confirmation is evaluated under the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.
