Hair retains a long-term record of drugs and alcohol by embedding biomarkers within its growing fibers. Hair collected near the scalp can reveal drug and alcohol use for up to about three months. Hair collection is straightforward, difficult to tamper with, and easy to send.

A 1.5-inch hair sample consisting of roughly 200 strands (approximately the thickness of a #2 pencil) near the scalp provides 100mg, the ideal quantity for testing. For EtG, additional testing, or tests exceeding 10 panels, a 150mg sample is suggested. It is advisable to weigh the sample using a jeweler’s scale. If scalp hair is not accessible, a comparable measure of body hair may be used. References to head hair mean scalp hair exclusively. Body hair encompasses all other hair types (e.g., facial, axillary).

Process Overview

The primary steps in laboratory drug test processing include Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial preparation of a sample for the laboratory system. This step involves ensuring the sample is sealed and shipped correctly, assigning a random Laboratory Accessioning Number (LAN), and inputting any additional data not covered by an electronic chain of custody.

Screening is a preliminary assessment for drug presence. While being a cost-efficient method for excluding drug use in most samples, any positive screens require confirmation to be valid in legal contexts. Samples testing presumptively positive in Screening need further confirmation.

When a sample receives a presumptive positive in Screening, extra hair is taken from the original sample for Extraction. This step involves removing drugs at much lower concentrations compared to other methods (e.g., urine or oral fluids), making hair drug testing especially challenging.

GC/MS, GC/MS/MS, or LC/MS/MS are used for confirming any positive screening result. All samples with presumptive positivity are pre-washed as necessary before confirmation. The comprehensive lab procedure from Accessioning to Confirmation is thoroughly reviewed under both the College of American Pathologists' (CAP) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
