What it is, How it works

When collected near the scalp, hair offers a drug and alcohol detection period of up to around three months. The collection is straightforward, relatively resistant to tampering, and easy to transport.

A 1.5-inch segment comprising roughly 200 strands near the scalp, similar in size to a #2 pencil, yields 100mg of hair, which is optimal for testing and verification. For EtG, additional testing, and/or more than 10 panels, a 150mg sample is advisable. It is recommended to weigh the sample using a jeweler’s scale. If scalp hair is unavailable, a comparable quantity of body hair can be used. When mentioned, head hair refers exclusively to scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of drug tests involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample is initially processed within a laboratory's system. This step involves confirming the correct sealing and shipment of the sample, assigning a random LAN (Laboratory Accessioning Number), and completing additional data entry as necessary when an electronic chain of custody is unavailable.

Screening involves a preliminary assessment for drugs. Though a cost-effective method for eliminating drug use suspicions in most samples, a positive screening result requires confirmation for legal admissibility. Samples that are presumptively positive during Screening must undergo further confirmation.

If a sample shows presumptive positivity during Screening, additional hair from the initial sample is prepared for Extraction. This stage involves extracting drugs from hair at significantly lower concentrations compared to techniques like urine or oral fluid, making hair drug tests notably complex.

Confirming positive screening results is done via GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples are pre-washed prior to confirmation as necessary. The entire laboratory process from Accessioning to Confirmation is conducted under the scrutiny of CAP (College of American Pathologists) Hair standards and ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
