Process Overview

The laboratory processing of a drug test result involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, the sample is initially processed into the laboratory system. This includes ensuring the sample is sealed and transported correctly, assigning a random Laboratory Accessioning Number (LAN), and completing any necessary data entry absent from an electronic chain of custody system.

Screening serves as a preliminary evaluation for substance abuse. While it is an economical method to exclude drug usage for most samples, any positive results require confirmation for court admissibility. Samples testing presumptive positive during Screening must undergo a secondary confirmation.

Should a sample test presumptive positive at the Screening phase, additional hair is extracted from the initial sample and prepared for Extraction. This stage involves isolating drugs from the hair at concentrations significantly lower than those in alternative methodologies (e.g., urine or saliva), making hair drug screening notably challenging to execute.

Confirmation of positive screening outcomes is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. All samples presumed positive are cleansed before confirmation if deemed necessary. The comprehensive laboratory process extending from Accessioning to Confirmation is evaluated under the CAP (College of American Pathologists) Hair designation as well as the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
