When taken near the scalp, it can indicate drug or alcohol use for roughly a 3-month period. Collecting the hair is straightforward, quite resistant to tampering, and easy to transport.

A 1.5-inch segment containing approximately 200 hair strands (comparable in size to a #2 pencil) nearest to the scalp yields 100mg of hair, which is perfect for both screening and confirmation. For EtG, additional assessments, or tests over 10 panels, 150mg of the specimen is advised. It's suggested to measure the specimen on a jeweler’s scale. If scalp hair isn't available, an equivalent portion of body hair can be used. When mentioning head hair, it specifically means scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The procedure in a laboratory for processing a drug test result involves four core steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the first step where a sample is incorporated into the lab’s system. This ensures the sample was properly sealed and sent, assigns a LAN (Laboratory Accessioning Number), and finalizes any additional details not captured by an electronic chain of custody system.

Screening acts as an initial rapid assay for detecting abused drugs. It's a cost-savvy approach to exclude drug use in the bulk of samples, though any positive Screening results require confirmation for legal admissibility. Samples that show presumptive positives in Screening necessitate a secondary confirmation.

When a sample tests presumptively positive during Screening, additional hair is taken from the initial specimen and prepared for Extraction. Here, drugs are extracted from hair at substantially lower concentrations than methods like urine or oral fluid analysis, highlighting why hair drug screening is particularly challenging to conduct.

Confirming any positive result from Screening is done through GC/MS, GC/MS/MS, or LC/MS/MS. All samples presumed positive are cleansed as necessary before confirmation. The complete lab process from Accessioning to Confirmation is scrutinized under CAP (College of American Pathologists) Hair standard and ISO / IEC 17025 accreditation.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field.
