A 1.5-inch piece of about 200 hair strands (approximately the thickness of a #2 pencil) closest to the scalp will yield 100mg of hair, the ideal quantity for both screening and confirmation. For EtG, additional testing, and/or panels exceeding 10, 150mg of the sample is recommended. Using a jeweler’s scale to weigh the sample is advisable. If scalp hair is not available, the same amount of body hair can be collected. Reference to head hair means scalp hair exclusively. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the preliminary processing of a sample into a laboratory's system. This step includes checking that the sample was properly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry absent from an electronic chain of custody system.

Screening is an initial rapid assessment for drugs of abuse. While it is cost-effective to rule out drug presence in many samples, a positive result requires confirmation for court admissibility. Samples deemed presumptively positive in Screening need secondary confirmation.

If Screening suggests a sample is positive, more hair is retrieved from the original specimen for Extraction. In this process, drugs are extracted from hair at a much lower concentration compared to other methods (e.g., urine or oral fluid), making hair drug testing one of the most challenging methodologies.

Confirmation of any positive screening result is done via GC/MS, GC/MS/MS, or LC/MS/MS. All samples showing presumptive positives are washed before confirmation as needed. The entire laboratory sequence from Accessioning to Confirmation is overseen under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 standards accreditation.
