A 1.5-inch segment of around 200 hair strands (comparable to the size of a #2 pencil) from the scalp will yield 100mg of hair, which is optimal for both screening and confirmation. For EtG, additional tests, and/or more than 10 panel tests, 150mg of the sample is suggested. We advise weighing the sample with a jeweler’s scale. If scalp hair is absent, an equivalent quantity of body hair can be gathered. By head hair, we specifically mean scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the initial integration of a sample into a laboratory’s system. This process includes confirming that the sample was correctly sealed and sent, allocating a random LAN (Laboratory Accessioning Number), and completing any remaining data entry not covered by an electronic chain of custody system.

Screening entails a fast preliminary check for substances of misuse. While Screening is an economical approach to eliminate drug usage in most samples, any positive screen result necessitates confirmation for legal acceptability. Samples preliminarily positive in Screening need a subsequent verification.

Should a specimen test presumptively positive during the Screening phase, additional hair is extracted from the initial sample and readied for Extraction. Here, drugs are isolated from hair at a considerably lower concentration than other techniques (such as urine or oral fluid), making hair drug screening the most challenging method to execute.

Positive Screening results undergo Confirmation using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptively positive specimens are cleansed before Confirmation as necessary. The comprehensive laboratory process from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
