Gathering a 1.5-inch segment of roughly 200 hair strands (about the diameter of a #2 pencil) near the scalp will yield 100mg of hair, ideal for testing. For EtG, add-ons, and/or assessments beyond 10 panels, obtaining 150mg is advised. We suggest using a jeweler’s scale for precise measurement. In the absence of scalp hair, an equivalent volume of body hair can be taken. When mentioning head hair, we refer solely to scalp hair. Body hair encompasses all other types like facial, axillary, etc.

Process Overview

The laboratory analysis of a drug test result comprises four core steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes integrating the sample into a laboratory’s system. This involves ensuring that the sample was sealed and dispatched correctly, issuing a random LAN (Laboratory Accessioning Number), and completing any missing data not submitted by an electronic custody system.

Screening involves a preliminary evaluation for drug presence. It’s a cost-efficient method to eliminate drug usage in most samples, but a positive result needs a follow-up confirmation for legal acceptability. Samples with initial positive results in Screening require a secondary confirmation.

Should a sample test positive during Screening, additional hair is extracted from the original sample and prepared for Extraction. Here, drugs are separated from the hair at lower concentrations compared to other techniques (e.g., urine or oral fluid), making hair screening notably challenging.

For any positive Screen, Confirmation is done using GC/MS, GC/MS/MS, or LC/MS/MS. Any samples that test positive are cleansed before confirmation as necessary. The entire lab process, from Accessioning to Confirmation, is reviewed under the CAP (College of American Pathologists) Hair classification and aligned with ISO / IEC 17025 credentials.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
