Process Overview

The laboratory processing of a drug test includes four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the preliminary stage of processing a sample within a lab’s workflow. This step ensures the sample was properly sealed and shipped, assigns a unique LAN (Laboratory Accessioning Number), and addresses any missing data that the electronic chain of custody did not capture.

Screening is an initial, rapid assessment for signs of drug consumption. While economical for eliminating drug usage in the bulk of samples, any sample registering a positive in this stage requires confirmation for court admissibility. Those samples that are tentatively positive in Screening need further confirmation.

When a sample indicates a tentative positive in Screening, additional hair is extracted from the original sample for Extraction. During this stage, drugs are extracted from hair at concentrations lower than those found in urine or oral fluid samples, making hair drug screening the most complex method to conduct.

Confirmation of any positive screening results is done through GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo washing before confirmation if necessary. The entire lab process, from Accessioning to Confirmation, is subject to review under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
