A 1.5-inch segment consisting of roughly 200 hairs (comparable to a #2 pencil) nearest to the scalp yields 100mg of hair, ideal for assessment and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, 150mg of hair is advised. We suggest using a jeweler's scale for precise measurement. If scalp hair isn't accessible, a similar quantity of body hair can be used. References to head hair signify scalp hair exclusively. Body hair encompasses other types, such as facial and axillary hair.

Process Overview

The laboratory processing of drug test results entails four essential steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning comprises the initial handling of a specimen within a lab's system, confirming the sample's proper sealing and shipping, assigning a unique LAN (Laboratory Accessioning Number), and completing any additional data that isn't covered by an electronic chain of custody system.

Screening involves a preliminary assessment for substance misuse. While cost-effective for eliminating drug presence in most samples, any positive screening result necessitates confirmation for court use. Samples indicating a presumptive positive in Screening require further confirmation.

Should a sample indicate a presumptive positive during Screening, additional hair is used from the initial sample and readied for Extraction. In this stage, drugs are separated from the hair at a considerably lower intensity than other methods (like urine or oral fluid), highlighting the challenge of hair drug screening.

Confirmation of any positive result from screening is achieved via GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo washing before confirmation if necessary. The complete laboratory process, from Accessioning to Confirmation, adheres to the CAP (College of American Pathologists) Hair criteria and ISO / IEC 17025 compliance.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
