Hair is easy to obtain, moderately resistant to tampering, and straightforward to transport.

A sample of 1.5 inches from about 200 hair strands (around the size of a #2 pencil) nearest to the scalp yields approximately 100mg, which is the optimal amount for both screening and verification. For EtG, additional tests, and/or panels above 10, 150mg of the hair sample is suggested. It is advisable to weigh the hair sample with a jewelry scale. If scalp hair is not available, a similar quantity of body hair can be used. When mentioning head hair, it specifically refers to scalp hair, while body hair includes all other hair types (such as facial or underarm hair).

Process Overview

The laboratory handling of drug test results involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning starts with incorporating a sample into the lab’s system, ensuring the sample was correctly sealed and transported, assigning a unique LAN (Laboratory Accessioning Number), and finalizing any additional data input not available via an electronic chain of custody.

Screening is the preliminary check for substance misuse. Even though screening is an economical approach to eliminate drug usage in most samples, a positive result needs verification to be legally valid. Any samples showing a presumptive positive result during screening require further verification.

Should a presumptive positive appear during the screening step, additional hair from the original sample is prepared for Extraction. In this phase, substances are drawn from hair at a concentration much lower than in other techniques (such as urine or saliva testing), which is why hair drug testing is the most intricate method to execute.

Confirmation of any positive screenings is done through GC/MS, GC/MS/MS, or LC/MS/MS. Every sample showing a presumptive positive result is cleaned before confirmation if necessary. The complete lab process from Accessioning through Confirmation is evaluated under both CAP (College of American Pathologists) Hair criteria and the ISO / IEC 17025 certification.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
