What it is, How it works

Hair retains a long-lasting record of alcohol and other drugs as biomarkers are encapsulated within its growing fibers. When taken near the scalp, it can reveal up to a 3-month period of detection for alcohol and other drugs. Hair is straightforward to gather, somewhat resistant to tampering, and simple to transport.

A sample of 1.5 inches from approximately 200 hair strands (about the diameter of a #2 pencil) near the scalp provides 100mg of hair, an ideal quantity for screening and confirmation. For EtG, additional tests, and/or panels exceeding 10, 150mg of hair is preferred. It is advisable to measure the specimen using a jeweler’s scale. If scalp hair is unavailable, an equivalent amount of body hair is acceptable. Scalp hair specifically refers to head hair, while body hair includes all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the sample's initial input into a laboratory’s system. This entails verifying the sample's proper sealing and shipping, assigning a random LAN (Laboratory Accessioning Number), and completing additional data entries that might not be provided via an electronic chain of custody.

Screening involves a preliminary, rapid assessment for drug abuse. Though Screening offers a cost-effective means to rule out drug use for most samples, a positive result requires confirmation to be admissible in legal settings. Samples that test presumptively positive require secondary validation.

If screening indicates a presumptive positive, further hair is extracted from the initial sample for the Extraction phase. Here, drugs are drawn from the hair at significantly lower concentrations than in other methods (e.g., urine or oral fluid), underscoring the complexity of hair drug screening.

Confirmation of any positive screening outcome occurs via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation as necessitated. The entire laboratory sequence from Accessioning to Confirmation is assessed under CAP (College of American Pathologists) Hair designation and compliant with ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
