What it is, How it works

The use of hair testing has emerged as an effective method for detecting drug and alcohol consumption. Hair captures a prolonged history of alcohol and drug use by embedding biomarkers within its growing fibers. When collected at the scalp, hair can offer a detection duration of up to roughly 90 days for alcohol and other drugs. The collection of hair is straightforward, challenging to tamper with, and simple to transport.

A specimen of 1.5 inches consisting of roughly 200 hair strands (approximately the size of a #2 pencil) nearest to the scalp provides 100mg of hair, which is the perfect sample for screening and confirmatory purposes. For EtG, add-ons, or tests beyond 10 panels, 150mg of hair is suggested. Using a jeweler’s scale for weighing the sample is advised. If scalp hair isn't available, a comparable quantity of body hair may be used. "Head hair" pertains to only scalp hair. "Body hair" includes all other hair types (facial, axillary, etc.).

Process Overview

The key phases of the laboratory process for a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the initial steps of processing the sample in a lab's system. This involves checking that the sample was appropriately sealed and shipped, assigning a unique LAN (Laboratory Accessioning Number), and completing any extra data entry not covered by an electronic chain of custody system.

Screening serves as an initial, rapid scan for drugs of abuse. While Screening is a cost-efficient method to exclude drug use in most samples, a positive screen requires confirmation to be legally admissible. Any samples showing a presumptive positive in Screening need further confirmation.

If a sample is identified as presumptive positive at the Screening phase, additional hair is taken from the initial sample for Extraction. Here, drugs are extracted from hair at a lower concentration compared to other methods (e.g., urine or oral fluid), making hair drug testing the most complex method to conduct.

Positive screening results undergo Confirmation using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives are washed before confirmation when necessary. The complete laboratory process, from Accessioning to Confirmation, is evaluated under the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation.

In this special issue of Drug Testing and Analysis, we aim to showcase the remarkable manuscripts presented at this significant event, shedding light on the latest research and highlighting the future directions in this field.

The impact of hair treatments on the oxidation/decomposition of drugs in hair was also explored, as the long-term fate of drugs incorporated in the keratin matrix. Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
