What it is, How it works

A 1.5-inch hair sample, approximately 200 strands (comparable to the width of a #2 pencil) cut nearest the scalp, yields 100mg of hair, ideal for both screening and confirmation. For EtG, additional tests, and/or panels exceeding 10, a 150mg sample is suggested. We advise using a jeweler's scale for accuracy. Should scalp hair be inaccessible, an equivalent quantity of body hair is acceptable. “Head hair” refers exclusively to scalp hair, while “body hair” includes all other types, such as facial or axillary.

Process Overview

The lab processing of a drug test result involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning covers the initial incorporation of a sample into a lab’s system, ensuring the sample was securely sealed and transported, assigning a random LAN (Laboratory Accessioning Number), and executing any supplementary data entry absent from an electronic chain of custody system.

Screening constitutes an initial rapid assessment for drugs of interest. Although cost-efficient for excluding drug use in most samples, a positive screen necessitates confirmation for legal acceptability. Samples showing presumptive positivity during Screening require further verification.

When Screening yields a presumptive positive, additional hair is extracted from the initial sample for Extraction. At this stage, drugs are extracted from hair at notably lower concentrations compared to other methods (e.g., urine or oral fluid), making hair drug analysis more complex to perform.

Any positive outcomes from Screening are confirmed using GC/MS, GC/MS/MS, or LC/MS/MS. Presumptively positive samples are cleaned before Confirmation as essential. The entire process from Accessioning to Confirmation is monitored under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
