For EtG, additional and/or tests exceeding 10 panels, 150mg of hair is advised. Using a jeweler's scale to confirm the weight of the hair sample is advisable. If scalp hair cannot be obtained, an equivalent volume of body hair is acceptable. Reference to head hair only pertains to scalp hair, whereas body hair encompasses all other hair types such as facial and axillary hair.

Process Overview

The processing of a drug test result in a lab involves four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning initiates with processing a specimen into the laboratory's database. This step verifies proper sealing and shipment of the sample, assigns a random LAN (Laboratory Accessioning Number), and completes additional data recording not captured by an electronic chain of custody.

Screening provides an initial rapid evaluation for substances of abuse. While Screening offers a cost-efficient way to rule out drug use for most samples, a positive result from Screening demands verification to be suitable for legal purposes. Samples deemed presumptively positive require further confirmation.

When a sample tests as presumptively positive during Screening, further hair is extracted from the sample for the Extraction step. Confirmation of all positive Screening results is achieved using GC/MS, GC/MS/MS, or LC/MS/MS techniques. Samples with presumptive positive outcomes undergo washing before confirmation if necessary. The complete laboratory procedure, from Accessioning to Confirmation, is evaluated under both CAP (College of American Pathologists) Hair and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
