When taken close to the scalp, hair offers approximately a 3-month detection period for alcohol and drugs. It is straightforward to collect, somewhat resistant to tampering, and easy to transport.
A 1.5-inch segment of about 200 hair strands (roughly the thickness of a #2 pencil) closest to the scalp will provide 100mg of hair, which is ideal for screening and validation. For EtG, additional components, and/or tests exceeding 10 panels, 150mg of the sample is suggested. It's advised to measure the sample using a jeweler’s scale. Process Overview
The four primary steps in laboratory processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.
Accessioning is the preliminary processing of a sample into a lab’s system. This includes checking that the sample is properly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and performing any additional data entry not covered by an electronic chain of custody system.
Screening involves an initial rapid test for drugs of abuse. While Screening is a cost-effective method to rule out drug use in most samples, a positive screen must be confirmed for legal admissibility. Any samples that test presumptively positive in Screening require further confirmation.
If a sample is presumptively positive at the Screening stage, additional hair is taken from the original specimen and readied for Extraction. In this phase, drugs are extracted from hair at a substantially lower concentration than in other methods (e.g., urine or oral fluid), making hair drug screening a challenging methodology.
Confirmation of any positive screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples are washed before confirmation, as necessary. The complete lab process from Accessioning to Confirmation is reviewed under both the CAP (College of American Pathologists) Hair designation and compliance with ISO / IEC 17025 standards.

Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.
The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
