For EtG, additional tests, and/or tests that exceed 10 panels, a 150mg sample is suggested. It's advisable to weigh the specimen using a jeweler’s scale. Should scalp hair be unavailable, an equivalent amount of body hair can be used. When discussing head hair, it refers exclusively to scalp hair. Body hair encompasses all other types of hair (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial integration of a sample into a lab's system. This phase includes confirming that the sample is sealed and shipped correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry not covered by an electronic chain of custody system.

Screening is a preliminary swift analysis for substances of abuse. Although Screening is economical in eliminating drug use in most samples, a positive result must be corroborated to be valid in a legal setting. Any samples that test presumptively positive during Screening require a subsequent confirmation.

If a specimen tests presumptively positive during the Screening phase, additional hair is sourced from the initial sample and prepared for Extraction. In this phase, drugs are extracted from hair at much lower concentrations than with other techniques (e.g., urine or oral fluid), which is why hair drug testing is considered the most challenging technique to execute.

Positive screening results undergo Confirmation via GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positive results are washed before confirmation as necessary. The entirety of the laboratory process from Accessioning to Confirmation is reviewed under the CAP (College of American Pathologists) Hair designation and accredited by ISO / IEC 17025 standards.
