What it is, How it works

When obtained near the scalp, hair can offer up to nearly a 3-month detection span for alcohol and other substances. Hair is straightforward to gather, a bit challenging to tamper with, and simple to transport.

A 1.5-inch segment of roughly 200 hair strands (approximately the width of a #2 pencil) nearest to the scalp will provide 100mg of hair, the optimal sample size for both screening and confirmation. For EtG, additional tests, and/or tests exceeding 10 panels, a 150mg sample is advisable. It's suggested to weigh the sample using a jeweler’s scale. In the absence of scalp hair, an equivalent quantity of body hair can be obtained. When mentioning head hair, we refer solely to scalp hair. Body hair encompasses all other hair categories (facial, axillary, etc.).

Process Overview

The four primary phases in the lab processing of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the initial handling of a sample into a lab’s system. This stage confirms the sample was properly sealed and shipped, assigns a random LAN (Laboratory Accessioning Number), and finishes any additional data entry not available from an electronic chain of custody system.

Screening constitutes a preliminary rapid inspection for drugs of abuse. Although Screening is an economical method to rule out drug consumption for most samples, a positive screening result needs to be confirmed for court admissibility. Any samples deemed presumptively positive in Screening necessitate a secondary verification.

In cases where a sample is presumptively positive during the Screening stage, additional hair is taken from the initial sample and made ready for Extraction. During this phase, drugs are isolated from hair at significantly lower concentrations than other methods (e.g., urine or oral fluid), which makes hair drug screening the most intricate method to execute.

Confirmation for any positive screening result is performed through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptively positive samples undergo washing before confirmation as required. The entire lab process from Accessioning to Confirmation is evaluated under the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
